Smoking is only permitted in designated areas outside the terminals.
2024, November update from Neil P.: Small clean smoking facility in the Mashonzha Lounge, which sits directly adjacent to the Emirates Business Class Lounge. Access to the lounge costs 580 rand for 4 hours if you are not in a premium class flight for a long number of airlines.
Update from Rene M., January 2023: There are private smoking lounges in the upper floor: Lounge 27 is the one I visited. Follow the signs to D Lounge and some others, go up the escalators and you'll see it. Consumption required, a coffee is 42 Rand (about 2.50$).
Update from Aeronaut, March 2020: There is a smoking room inside the pizza shop, I think you need to buy something in the overpriced place to access.

area inside the restaurant Diner "Piece A Pizza", in front of the
small Big 5 duty free, direction to the gates A5-A30
There is a smoking lounge on the first floor, it is run by British American Tobacco.
In reply to the next comment from Randolf J. Sept 2018: It is sketchy, if someone is pressing you for a tip, but I can definitely tell you, that tips in other currencies are NOT illegal.
There is a smoking area in Terminal A
airside, but it closes at 8pm (entrances are refused by 7:30pm). A
word of caution: after closure, airport employees will offer you to
smoke in a corridor right beside the smoking area. They’ll then
press you for a tip “in any currency” as you’re leaving. Tips in
other currencies can be illegal in South Africa, so be warned this
is a very sketchy deal.
