Smoking is only permitted in designated areas outside the terminals.
Update: Latest report suggests that you can smoke anywhere outside the terminals and you are not restricted to smoking areas. Thanx to SP for this information.
2024, June update from Sanj: Smoking Terrace still open. can be found in Cibeles lounge near gate B26. Very good open space lots of chairs. Easyjet and Ryanair fly from here. Priority Pass friendly.

Update from BarryOS, March 2020: Was in T1 for a few hours and found the terrace in the VIP area is still available for smokers and loungers alike. either traveling with business will let you in for free or a fee of 36 euro to use the lounge. maximum stay is 4 hours prior to departure.

that the Smoking outdoor area inside the T1 VIP Lounge is still
open.
VIP Sala lounge has smoking. Anyone flying
business or anyone who has priority Pass can get in.

Update from MK, June 2023: Smoking section in Madrid airport is incredible. Outdoors but covered. Tables and chairs. Vending machines. High end food truck. Charging stations. Plants. never seen better.

Update from Piotr, July 2022: I can confirm: there is open air smoking area after security check, it is located near entrance to the gates J40-J9 (down floor) or if you landed in Madrid, on the way to baggage claim.
